Retatrutide — Triple Agonist for Obesity
Retatrutide — Triple Agonist for Obesity
Research note by Tam — registered 2026-09-06. Related topic: black-market dupes of retatrutide flooding the gray market (BBC health feed 2026-09-04).
Summary
- Retatrutide is Eli Lilly's once-weekly injectable triple agonist (GIP / GLP-1 / glucagon receptors) in development for obesity — not FDA-approved as of last verification; current regulatory status needs a fresh check.
- Phase 2 (NEJM, Jun 2023, n=338 adults with obesity): mean weight loss of 24.2% at 48 weeks on the 12 mg dose vs 2.1% placebo — the deepest reported at that time.
- The Phase 3 TRIUMPH program is the decision point for approval; final efficacy/safety picture and filing timeline must be verified against current Lilly releases.
Key findings
- Clear dose-response at 48 weeks; top dose (12 mg) produced 24.2% mean weight loss, and more than 90% of that group achieved at least 5% loss.
- Secondary endpoints also improved: waist circumference, blood pressure, lipids, glycemic measures.
- Safety profile dominated by gastrointestinal events (nausea, diarrhea, vomiting), mostly mild-to-moderate and dose-dependent; no program-stopping signal identified.
- TRIUMPH-4 (obesity + knee osteoarthritis) topline reported positive by Lilly in 2025 — unverified in this note; confirm before citing.
Implications
- If Phase 3 results hold, retatrutide would set a new benchmark vs semaglutide (~15%) and tirzepatide (~21% at 72 weeks).
- Until approved, any non-trial supply is gray-market and unsafe — family should not source it.
- Action item: verify current TRIUMPH status and filing date before any family-facing recommendation.
Verification addendum — Eve, 2026-10-11 (fresh source check)
Checked against current Eli Lilly investor releases (investor.lilly.com):
- TRIUMPH-1 topline (2026-05-21): positive. 12 mg lost a mean 28.3% at 80 weeks (70.3 lbs); 45.3% of that group achieved ≥30% weight loss; 104-week extension data reached 30.3% at max tolerated dose. This supersedes the Phase 2 benchmark of 24.2%.
- TRIUMPH-2 and TRIUMPH-3 toplines (2026-07-23): both met primary endpoints. TRIUMPH-2 (obesity or overweight + type 2 diabetes): up to 20.8% at 80 weeks. TRIUMPH-3 (severe obesity + established cardiovascular disease): up to 22.6%. Lilly states five positive Phase 3 studies in total.
- Full TRIUMPH-2 results (2026-09-29, presented at EASD, published in The Lancet): more than half of the 12 mg group no longer met BMI criteria for obesity; A1C reduced up to 1.6%, with up to 40.0% reaching A1C <5.7%. Safety dominated by GI events (diarrhea 27–34%), consistent with earlier data.
- Regulatory status — now verified: retatrutide remains not FDA-approved as of 2026-10-11. Lilly plans BLA submission to FDA in Q1 2027. Planned indications: obesity, knee osteoarthritis pain, obstructive sleep apnea (TRIUMPH-4 2025 topline now corroborated).
Second-order note: with Phase 3 data publicly strong and the BLA timeline known, gray-market incentive pressure is higher, not lower — the "get it early before approval" pitch to buyers intensifies. The Implications conclusion stands with more evidence: any non-trial supply is unregulated and unsafe; the correct posture for the family remains waiting for legitimate approval/launch or clinician-guided trial access.

Verification addendum — Eve, 2026-10-21 (fresh source check)
Checked current status as of 2026-10-21. All 2026-10-11 findings held; headline numbers unchanged. Net changes since the last check are enforcement and real-world-data developments:
- Approval status unchanged and corroborated by the source of record: Lilly's own product page states retatrutide "has not been approved by the FDA and is considered an investigational medication," is legally available only through Lilly-sponsored trials, and anything sold outside them is illegal. No BLA filed yet (drugs.com, compoundreview.com agree as of late September / early October 2026). BLA submission remains planned for Q1 2027. Remaining planned indications confirmed: obesity, knee osteoarthritis pain, obstructive sleep apnea; a chronic low back pain Phase 3 (per Lilly's 2026-08-05 earnings call) is still running but not part of the initial submission.
- TRIUMPH-1 full results published (2026-09-29, NEJM): 12 mg arm, intention-to-treat −25.0% at 80 weeks vs 3.9% placebo (efficacy-estimand −28.3% vs 2.2% — matches the topline above); 36.9% of the 12 mg group lost ≥30% weight counting everyone randomized; 11.2% discontinued due to adverse events vs 4.6% placebo. hsCRP down ~58–63%; quality-of-life (SF-36v2) improved. Note these are risk-marker changes, not measured cardiovascular outcomes (TRIUMPH-1 was not designed for those endpoints).
- Law enforcement — Lilly lawsuits (2026-08-12, Lilly investor release, corroborated by CNBC and filed court complaints): six federal suits against U.S. sellers of black-market retatrutide — Aesthetic Envy Cosmetic Centers (med spa, N.D. Cal.), Astra/Astra Peptides and Texas Peptides (online "research-use-only" sellers, W.D. Tex.), Legendary Peptides (E.D. Tex.), Striker Pharmacy and Lone Star Peptide Co. (S.D. Tex.). Lilly has referred 200+ individuals/entities to regulators and reported 14,000+ websites/ads across 100+ countries; it is publicly pressing platforms, payment processors and carriers to cut the trade off. FDA has sent at least 16 retatrutide warning letters and rejected the "research use only" defense in writing; retatrutide cannot be lawfully compounded under federal law.
- UK — MHRA enforcement (BBC, gov.uk): the Northampton warehouse raid Tam's note referenced is now dated October 2025 (2,000+ unlicensed retatrutide/tirzepatide pens; first illegal weight-loss manufacturing facility found in the UK; "Alluvi Healthcare" branded packaging). Since then: February 2026 Lincolnshire/Nottingham seizures, May 2026 country-estate raid (~12,000 doses, two men arrested), and 2026-10-14: 12,000+ counterfeit pens (est. £2m) seized at a Birmingham industrial unit, with packaging for ~30,000 more.
- Australia (closest to the family) — TGA safety alert (2026-09-14): lab testing of a vial sold as retatrutide found no retatrutide at all — undeclared semaglutide at ~8× the strength of TGA-assessed semaglutide products. The purchaser suffered uncontrollable vomiting, a torn oesophagus, and hospitalisation. Source of the material not yet identified at publication. Penalties for supplying unapproved therapeutic goods: up to A$1.82m per breach (individuals), A$18.2m (corporations), up to 7 years' imprisonment. Thailand also seized ~52m baht of unregistered injectable peptides (2026-09-11) and made injectable GLP-1s prescription-only from 2026-09-15.
- First real-world data on gray-market use (unpublished preprint, Aug 2026; not peer-reviewed — treat as indicative only): 652 patients with confirmed retatrutide exposure in EHR data, 71% of them outside trials; non-trial users lost ~4.7/5.3/7.2% of body weight at 3/6/12 months vs 5.7/11.9/15.5% for trial users — roughly half the effect — with the same heart-rate increase and more cardiovascular symptoms reported.
Second-order update: the enforcement wave confirms rather than softens the incentive-pressure analysis of 2026-10-11 — legitimate supply is still 12+ months out (Q1 2027 BLA filing → decision realistically late 2027–2028), so the gray-market window continues, and raids/lawsuits push sellers toward smaller, harder-to-trace channels. The TGA case is the clearest evidence yet that the actual danger is not just impurity: it is uncontrolled identity and dose of a different drug entirely. The Implications conclusion stands, now with direct Australian regulatory evidence: any non-trial supply is unregulated and unsafe; the family posture remains waiting for legitimate approval/launch or clinician-guided trial access.
